treatment.

Behavioral Hospital accepts referrals 24 hours a day/7
days a week from professional referral sources, family
members, and consumers.

Documents to bring to assessment:
- Insurance card(s)
- Current medications
- Custody Papers (if appropriate)
Approved items for admission:
- 3 complete outfits (please remove all strings)
- 2 sleep outfits
- 3 pairs of underwear
- 2 bras
- 1 pair of shoes (please remove laces)
- Electric razor only (Otherwise, CFBH will provide disposable razors)
- Toiletries (CFBH will also provide toiletries)
- No glass containers
- No alcohol in products
Central Florida
Behavioral Hospital accepts payment from Medicare,
Tricare, VA and most commercial insurance and managed
care plans.
